Neutral theory: applicability and neutrality of using generic health-related quality of life tools in diseases or conditions where specific tools are available

Abstract Background Health-related quality of life (HRQoL) tools are limited by the indicators included in construct and variation interpretation different researchers. Neutral Theory describes ideal that includes all relevant and, therefore, complete accuracy, or neutrality. can thereby provide framework to develop test constructs. To assess application Theory, neutrality generic (SF-36 EQ-5D) at measuring HRQoL was compared disease/condition-specific tools, with latter considered surrogates for construct. Methods Full descriptions published on PubMed (to 01-Jul-19) were sourced. For each tool, number items without a direct match within SF-36 EQ-5D recorded sensitivity/specificity calculated. Results The did not achieve both > 50% against any 163 identified. At 20% prevalence poor HRQoL, false positive rate (FPR) 75% but two six EQ-5D. No concordance 18% (2/11) results studies versus (no data vs EQ-5D). non-rare, disease-specific discordant 30% (25/84) 35% (23/65) EQ-5D, corresponding 36% (24/66) 16% (5/31). Conclusions Generic appear poorly correlated which indicates adoption development assessment could improve their relevance, utility economic evaluations health interventions.
